As you can see by the pics this tree has been nearly pushed over by other plants that were crowding it badly. The main trunk is only about 1ft off the ground and is almost horizontal. It's now only producing branches in one direction and I'm not sure whether to prune it back hard, forego the fruit this year and hope the tree starts shooting from the main branch again. Another problem is if I do allow it to shoot from the main trunk will it be able to hold any new growth long term? Other than that it's a healthy tree, any ideas would be much appreciated:)

Have you tried staking it to straighten it upwards?
You can start by taking the weight off the main trunk and remove that lower branch right off!
Then stake it and apply a bit of pressure to pull the trunk back! Don't lift it all at once, but a bit this year, and then more next until it is more upright!
Keep tightening the pressure each year until the tree stands more upright! You can prune back the next branch as you go along!
It will be a long term correction!
Hopefully side branches will form and you can train them upwards!
It will never be completely straight though!
